Outcome
Our dashboards were initially developed as part of the Out-of-Hospital Care Models (OOCHM) for people experiencing homelessness, funded by the Department of Health and Social Care. This initiative aimed to reduce street discharges, readmissions, and unplanned hospital visits while improving health and well-being. The dashboards facilitate routine data collection, analysis, and trend comparison, providing valuable insights for local authorities and service providers.
